Quality Clauses
(Addendum to Flexial Standard P.O. Terms and Conditions)

Quality System: Vendor must be registered to the ISO9001:2000 Quality management System Standard. Vendor’s registrar must be ANSI:Rab Accredited (or non-U.S. equivalent for non-domestic suppliers). A copy of the registration document must be forwarded to Flexial procurement via Email* |

Calibration System: The contractor’s calibration system shall conform to the requirements of ANSI/NCSL Z540-1-1994 (or later) or ISO10012:1992 (or later), or ISO17025 (current revision) and is subject to review and approval at all times by Flexial. A copy of the registration document must be forwarded to Flexial procurement via Email*. |

Right of Access: Flexial and its customers reserve the right to review, verify, and participate in the contractor’s quality assurance systems and associated manufacturing processes, including inspection and testing of any work related to this contract, at the supplier’s premises. This activity, if acted upon, shall be performed in such a manner as to not disrupt normal processing and shall be conducted on a non-interfering basis. |

Source Surveillance: Source surveillance shall be conducted by Flexial or Flexial’s representative at the contractor’s facilities or where designated in this contract prior to shipment. Inspection and \ or testing shall be performed by the contractor and shall be subject to witness by Flexial representative(s). |

Certified Material Test Reports: One copy of test reports indicating chemical composition and \ or actual physical properties identifiable to each lot, batch, or heat treat lot shall accompany each shipment, and shall be validated by an authorized contractor’s representative. The Certified Material Test Report shall be forwarded to Flexial prior to receipt of each shipment via Email*. |

Test Reports: Actual Functional Test Reports referencing contract number (PO Number), contractor’s name and address and (if applicable) independent laboratories’ name and address, Flexial part number and revision, part name, serial number (if applicable), test date, and lot number \ date (if not serialized), must accompany each shipment, and shall be validated by an authorized contractor’s representative. The Functional Test Report shall be forwarded to Flexial prior to receipt of each shipment via Email*. |

Physical Inspection Reports: Physical (dimensional) Inspection Reports referencing contract number (PO Number), contractor’s name and address, Flexial part number and revision, part name, serial number (if applicable), test date, and sample number (if not serialized), must accompany each shipment, and shall be validated by an authorized contractor’s representative. The sample size shall be 100%. The inspection report shall include all specifications identified on the Flexial drawing, and shall record the specification and tolerance, actual measured result, measurement method, and disposition for each characteristic. The Inspection Report shall be forwarded to Flexial prior to receipt of each shipment via Email*. |

Certificate of Conformance: The contractor shall submit with each shipment a Certificate of Conformance that shall be dated and shall bear the signature and title of an authorized contractor’s representative. The Certificate of Conformance shall reference contract number (PO Number), contractor’s name and address, Flexial part number and revision, part name, serial number(s) (if applicable), test date, and quantity certified. |

Material Traceability: The contractor shall have established a system for the identification, traceability, and control of materials, parts, and assemblies from acquisition through fabrication, assembly, test, and delivery. The system shall provide for the ready identification of suspect lots when individual items are found to be discrepant. |

Flowdown of requirements to sub-suppliers: Flexial’s sub-suppliers are required to flow down all quality clauses listed on this purchase order to sub-suppliers of vendors supplying product to Flexial where this clause is referenced. |

Other file formats may be accepted with authorization
in advance from Flexial Purchasing. The Email address
for document submission shall be noted on the Purchase
Order.
